If you request your
repeat prescription by writing to us you
must include you name, your address,
your date of birth, how you would like
to collect your prescription and a full
list of items required.
You can write your request at reception,
deliver your letter by hand or send it
to us in the post.
Please remember to allow extra days for
possible delays by Royal Mail.

envelope with your written prescription
request, we can post you prescription
